Please mail your completed sponsor confirmation form with a check made payable to Summer Celebration, Inc. to Summer Celebration Sponsors, 301 E. Irving Park Road, Streamwood, IL 60107.  If you have any questions or need more information, please call (630) 736-3803 or send an email to mayor@streamwood.org.  If your company requires an invoice, please return your completed sponsor form and we will be happy to issue an invoice.
